Monitoring seat feature usage among wheelchair users.

To help manage pressure ulcers and adjust posture, power wheelchairs can be equipped with seat tilt and backrest recline features. Clinicians usually prescribe these features for wheelchair users who are vulnerable to pressure sores or have difficulty changing postures, but little data has been collected to determine whether or not, and what extent, these features are used. This study examined how www.easymedonline.com users use power wheelchair seat features such as seat tilt, backrest recline, and seat elevation, during typical daily activities. A Seat Feature Data Logger (SFDL) was attached to 11 subject’s wheelchairs for 10-14 days to gather data regarding daily usage of these features. The results showed that while subjects did not always use large angles of tilt and recline as many clinicians recommend, these features were used frequently and their use resulted in lower peak pressures. The information collected by the SFDL allows for a novel and quantitative description of the effectiveness and use of power seat features, and can also be used as a clinical compliance tool

Power mobility device provision: understanding Medicare guidelines and advocating for clients

The Centers for Medicare and Medicaid (CMS) issued a new national coverage determination (NCD) for mobility assistive equipment (MAE) including power wheelchairs, www.easymedonline.com in May 2005. Epidemiology of Medicare abuse: the example of power wheelchairs.

OBJECTIVES: To determine the effect of neighborhood ethnic composition on medical scooters prescriptions. DESIGN: The 5% noncancer sample of Medicare recipients in the Surveillance, Epidemiology and End Results (SEER)-Medicare linked database, from 1994 to 2001. SETTING: SEER regions. PARTICIPANTS: Individuals covered by Medicare living in SEER regions without a cancer diagnosis. MEASUREMENTS: Individual characteristics (age, sex, ethnicity, justifying diagnosis, and comorbidity), primary diagnoses, neighborhood characteristics (percentage black, percentage Hispanic, percentage with <12 years education, and median income), and SEER region. RESULTS: The rate of power wheelchair prescriptions was 33 times greater in 2001 than in 1994, with a shift over time from justifying diagnoses more closely tied to mobility impairment, such as strokes, to less-specific medical diagnoses, such as osteoarthritis. In multilevel, multivariate analyses, individuals living in neighborhoods with higher percentages of blacks or Hispanics were more likely to receive power wheelchairs (odds ratios=1.09 for each 10% increase in black residents and 1.23 for each 10% increase in Hispanic residents) after controlling for ethnicity and other characteristics at the individual level. CONCLUSION: These results support allegations that marketers promoting power wheelchairs and handicap scooters have specifically targeted minority neighborhoods

The effect of providing power mobility on body weight change

OBJECTIVE: To investigate whether the provision of power mobility would have an effect on body weight in adults who were first-time qualifiers for power mobility. DESIGN: This was a retrospective observational study of consecutive subjects, who served as their own controls. Three weight groups (12 mos before, at, and 12 mos after the power mobility evaluation) were evaluated with repeated-measures analysis of variance (ANOVA). The weight changes on subjects in different age groups (45-54, 55-64, 65-74, and >74), in geriatric vs. nongeriatric groups, and in different body mass index (BMI) groups were analyzed. RESULTS: Eighty-nine subjects met the inclusion criteria. They were obese (49.4%), and most of them were geriatric (64%). Congestive heart failure (30.34%) and chronic obstructive pulmonary disease (22.47%) were the two main presenting diagnoses. The repeated-measures ANOVA showed no significant weight change in the three studied weight groups. Similar results were seen in the age and BMI subgroups. CONCLUSIONS: There was no statistically significant weight change in adults who were first-time qualifiers and who used power mobility for 1 yr.

Exercise responses and adaptations in rowers and spinal cord injury individuals.

Elite rowers (ROWERS) and those who have had a spinal cord injury (SCI) are different physically in many realms. Both have physical activity histories that affect their lower-extremity extensor muscles in a dramatically different fashion. ROWERS can sustain a 500-W power output during their 5- to 6-min race. After a complete SCI, a 75-W power output might be achieved during a VO(2peak) test. A preliminary study on the impact of pushrim-activated power-assist wheelchairs among individuals with tetraplegia

OBJECTIVES: This preliminary study was to evaluate the impact of pushrim- activated
power wheelchairs (PAPAWs) on mobility, community participation, satisfaction, and psychosocial impact among individuals with tetraplegia. DESIGN: Fifteen manual wheelchair users with tetraplegia completed a 4-wk protocol including a 2-wk trial where they used their personal wheelchairs for mobility and a 2-wk trial where they were provided with a PAPAW and could choose either wheelchair for mobility. The mobility levels with both wheelchairs were recorded by a datalogger. Participants completed daily questionnaires regarding community participation and medical scooters their satisfaction with the wheelchair used. The Psychosocial Impact of Assistive Devices Scale was used to compare the psychosocial impact of PAPAWs with their personal wheelchairs. RESULTS: Participants chose to use PAPAWs and their personal wheelchairs at a similar frequency in the PAPAW trial. The distances traveled with both wheelchairs were not statistically different (P = 0.09). Participants traveled significantly faster with PAPAWs (P = 0.03). PAPAWs did not result in significantly greater community participation, satisfaction, and psychosocial impacts than the personal wheelchairs. CONCLUSIONS: PAPAWs could be a viable mobility option for individuals with tetraplegia, which can provide independent mobility especially for outdoor activities. A user’s preference, life style, physical conditions, transportation issues, and environmental factors should be considered in prescribing such a device.

Effects of graduated compression stockings on cardiovascular and metabolic responses to exercise and exercise recovery in persons with spinal cord injury.

OBJECTIVE: To investigate whether reporting blood redistribution by means of graduated elastic stockings affects exercise and postexercise responses in people with spinal cord injury (SCI). INTERVENTIONS: Subjects performed 2 maximal power wheelchairs exercise tests 1 week apart, in random order and under a counter-balanced design. One test was done with and the other without graduated elastic stockings (21 mmHg). MAIN OUTCOME MEASURES: Blood lactate, blood pressure, heart rate, maximal power output, and oxygen consumption (Vo2). RESULTS: Postexercise venous lactate concentration was reduced in SCI subjects with lesion levels below T6 while wearing graduated elastic stockings during both exercise and recovery (10.9+/-3.9 mmol/L vs 12.5+/-4.6 mmol/L, P<.05). There were no significant differences in submaximal and maximal values (heart rate, Vo2, power output) between subjects tested with and without graduated elastic stockings. CONCLUSIONS: Wearing elastic stockings affects postexercise responses by decreasing lactate concentration in well-trained, low-level paraplegic patients after a maximal exercise. The relatively low pressure generated by the stockings may not, however, influence the venous system enough to produce improved performance and cardiovascular responses

The Hephaestus Smart Wheelchair System

Demographic and socioeconomic factors associated with disparity in wheelchair customizability among people with traumatic spinal cord injury

OBJECTIVES: To determine if a standard of care for wheelchair provision exists within the participating centers and if there is disparity in wheelchair customizability among the study sample. DESIGN: Convenience sample survey. SETTING: Thirteen Model Spinal Cord Injury Systems that provide comprehensive rehabilitation for people with traumatic spinal cord injury (SCI) and that are part of the national database funded through the US Department of Education. PARTICIPANTS: A total of 412 people with SCI who use wheelchairs over 40 hours a week. INTERVENTION: Survey information was obtained from subjects via telephone and in-person interviews and from the national database. Collected information included age, race, education, level of injury, and wheelchair funding source. MAIN OUTCOME MEASURES: Number and type (manual or power) of wheelchairs. Wheelchair customizability as defined by design features (eg, adjustable axle position, programmable controls). RESULTS: Ninety-seven percent of manual wheelchair users and 54% of power wheelchairs users had customizable wheelchairs. No power wheelchair user received a wheelchair without programmable controls. Minorities with low socioeconomic backgrounds (low income, Medicaid/Medicare recipients, less educated) were more likely to have standard manual and standard programmable power wheelchairs. Older subjects were also more likely to have standard programmable power wheelchairs and www.easymedonline.com. CONCLUSIONS: The standard of care for manual wheelchair users with SCI is a lightweight and customizable wheelchair. The standard of care for power wheelchairs users has programmable controls. Unfortunately, socioeconomically disadvantaged people were less likely to receive customizable wheelchairs.

Comparative evaluation of chargers for wheelchair gel cell batteries

This study was undertaken as part of our ongoing work to upgrade wheelchair quality. Laboratory tests were conducted to determine which battery chargers would ensure that the user’s batteries are fully charged each night and also ensure that the battery lifetime was not diminished. Five commercial chargers were tested on a standardised pair of gel cell batteries of a type commonly used to power wheelchairs and medical scooters. Results of this study indicate a clear preference between chargers. As a result, we have changed the chargers we purchase. Responses to the acquisition and use of power mobility by individuals who have multiple sclerosis and their families

PURPOSE: To develop an understanding of family members’ reactions to the acquisition of medical scooters by persons with multiple sclerosis (MS) from the perspectives of the end users and their family members. METHODS: Data were obtained through semistructured interviews with seven persons with MS using or considering power mobility and four of their family members. RESULTS: Three major themes emerged from the data. Recognizing the Need for handicap scooters resulted from the interaction between multiple sclerosis progression and the participants’ desired performance. Family decision making and communication, insurance funding and approval, and the physical and social environment were just some of the factors affecting the process of Deciding and Obtaining the Power Mobility. Using Power Mobility resulted in positive, negative, and neutral outcomes. CONCLUSIONS: This study uncovered an overall lack of resources and some issues within the family environment, such as communication and decision making, both of which can negatively impact the acquisition and use of power mobility for persons with MS. Understanding these challenges may assist occupational therapists to facilitate this transition with their clients.
